Job Description:

This position transports clients in a safe manner to/from medical appointments, court, etc. in ensuring quality of service delivery. The position supports the direct care staff and department(s) in ensuring that Agency guidelines are followed in residential settings, and in the medical area. This position will be part of ratio during first shift for youth/staff ratio compliance and in ratio for emergency/crisis situations as assigned by management.

1.Transportation of residents to medical appointments and court(s) and provision of feedback and paperwork to the Supervisor.
2.Knowledge of whereabouts and direct supervision of all adolescents in transportation, including documentation of adolescent’s presence on the tracking chart every 15 minutes, or sooner, as situations arise, and safety plans dictate or warrant for client safety.
3.Take appropriate action in handling crisis situations in accordance with agency policy / guidelines and rules; this may include therapeutic interventions.
4.Oversee adolescents and make effort to see that agency policies / rules are enforced.
5.Be aware of each adolescent’s daily schedule; keep track of vital information and share with staff on the unit when necessary.
6.Communicate effectively both verbally and in writing with a broad variety of people, including other team members and management.
7.Monitor medical status of clients and escort adolescents to the doctor / hospital when indicated.
8.Share information with Nursing Supervisor, and other staff members, as needed, and keep daily records concerning adolescents in the communication log as well as in each client’s individual log.
9.Maintain appropriate professional boundaries and confidentiality. Serve as a positive role model and use collaborative decision-making skills.
10.Exercise tact, discretion, and good judgment in working with a variety of people, and exhibit a respectful, caring, and understanding attitude toward all adolescents and their families.
11.Facilitate Group Sessions with adolescents according to the structure and treatment schedule for the program.
12.Attend team meetings and work with other staff as a team to carry out the mission of the Agency.
13.Display openness to feedback and supervision.
14.Monitor, log and dial all phone calls placed by adolescents, and monitor their phone time.
15.Supervise and transport adolescents as required by the program.
16.Other duties as assigned

High School diploma and one year of experience with children and youth.
•A clear driving record, law enforcement record, and drug screen.
•Must be minimum 24 years old in order to be covered by company’s insurance policy.

Physical Requirements (BFOQ):
•Ability to provide satisfactory level of visual and auditory supervision of adolescents
•Physically capable of meeting Handle with Care (restraint) requirements
•Ability to stand or sit for extended periods
•Bend, stoop, squat and climb steps/stairs as needed
•Ability to lift up to 40 pounds
•Able to walk and/or run at a level that keeps pace with clients
•Ability to remain conscious and alert if/when scheduled to work late night/early morning hours
